STEP by STEP Directions:

 

1.  Get your Physical 
2.  Create an account (if you don't have one yet) at homecampus.com and UPLOAD A COPY OF your completed physical form and insurance card
3.  BRING THE ORIGINAL PHYSICAL FORM TO THE SANTEE ATHLETIC DIRECTORS.
4.  IF YOU WEAR GLASSES OR CONTACTS:  YOU MUST HAVE A HARD COPY OF EYE EXAM GLASSES PRESCRIPTION
5.  UPLOAD PICTURE OF INSURANCE CARD ONLINE
6.  PRINT OUT AND SIGN PAPER OF COMPLETION AND RETURN TO YOUR Coach who will turn in to Martinez
 
NOTE:  Any student participating in Athletics must have a 2.0 G.P.A. or “C” average.

 

 

To join any LAUSD Athletic program, you will need to submit a completed physical form, signed by a doctor.  The form must be uploaded onto athleticclearance.com AND you must turn in the original physical form to Santee's Athletic Directors.
